Here at Rexburg Medical Clinic, we have a program in place to help you manage your ongoing chronic care needs. 2. Contacting your various healthcare providers and coordinating their efforts
3. Participating in monthly call-ins with you, to assess your progress, answer your questions, and keep things moving smoothly
4. Providing you with essential education regarding your particular chronic condition, so you’re armed with the knowledge needed to deal with it
5. Making you aware of the many resources available to you in this community
